Immunization Requirements

Massachusetts State Law requires that all incoming students provide verification of specific immunizations. Documentation of immunizations must be received prior to the start of classes. 

Your health care provider should complete the Immunization History Form and administer any immunizations that have not yet been received. You should read and return the Meningoccal Information and Waiver Form if you are not receiving this immunization. Forms must be completed in English.

Forms for new students entering in Fall 2013 should be submitted no later than Thursday, August 1st.

Please note: additional forms or records WILL NOT BE ACCEPTED. Please do not submit any other documents regarding your health history. Additional documents will not be returned and will be destroyed.
