Summer Dance Application Instructions (PDF)

To apply to Summer Dance '08 open the application through the link above, print it out, and then complete it following these instructions:

1. All applicants must be 16 years of age or older. All applications must include two letters of recommendation: one from your current dance instructor and one from a guidance counselor or academic advisor.
2. Please provide your name, gender, social security number, date of birth, telephone number, email and home address.
3. Please indicate which session(s) you will be attending (First, Second, or Both).
   Please indicate whether you plan to live on campus or commute.
   If you're over 18, please indicate if you wish to participate in the planned activities. These activities (see the "Activities" section of the Policy booklet) are mandatory for students under the age of 18 who are living on campus. Participation requires payment of the Activities fee, per session.
4. Please indicate your perceived technical level in both ballet and modern dance.
5. Please rank your choices (1-3, 1 as highest) of elective classes for the appropriate session. One elective class is included in your tuition, and we will try to place students according to their first choices. There is an additional fee for taking an additional elective class, per session.
6. Please list your most recent and previous dance training. Be as specific as possible, or attach your résumé. Please also indicate if and when you've studied at The Boston Conservatory before, and if you intend to apply to The Conservatory's undergraduate program.
7. Section 7 must be completed to process this application. We require basic medical history and medical treatment authorization.
8. Section 8 must be completed to process this application. Please fill out and sign this section. If you are under 18 years of age, your parent or guardian must also sign this waiver.

Please mail the application with a nonrefundable application fee of $65 and two recommendations to:

Checks should be made payable to The Boston Conservatory.

Upon acceptance we require a tuition deposit of $500 per session. This deposit will be credited toward the final amount you owe for tuition and fees.
